This Document is to grant consent for your child to access and use the internet. This form must be signed and checked as to whether or not permission is granted.
Page 2 is the computer policies and responsibilities Form. This must be signed by parent and student before a student is allowed to use a computer in the school.
- La Care Form
This is a registration form for LaSalle Academy's after school extended hours program. This form is for drop-in care as well as scheduled care.
- Field Trip Form
This is a generic form that you can print and sign in case your child lost theirs. It is not specific to the trip or class and must be updated with your specific information.
- Request for Excused Absence from School for a Preplanned Educational Tour or Trip
Complete this form when planning an educational trip or tour during which the student will be absent from school. This must be completed and returned to school at least 5 days before the start of the intended trip.
- Transportation Request Form
Use this form to request transportation from your public school district to La Salle Academy.
- Communication with Teacher
You may use this form to report a student absent, late or if they are leaving early. You may also use it for other brief communication with the teacher.
